A First-of-Its-Kind Tool for Private Duty Nursing 

For over 16 years, Team Select Home Care has supported medically complex children and adults through Private Duty Nursing (PDN) — a model built on compassion, skill, and one-on-one care. But even the most experienced nurses face limits: human observation can only see what happens during one shift. 

CareSightAI changes that. 

Built internally by Team Select, CareSight AI is the result of a true cross-functional collaboration between our Data Analytics, Information Technology, and Clinical Operations teams — with direct clinical oversight from our Chief Clinical Officer and clinical leaders across the nation. Partnering with Cuesta Partners for strategic technology consulting, Team Select developed a platform that continuously analyzes patient data from our electronic medical record (EMR) system, transforming it into actionable insights that help identify early signs of respiratory change.

The tool identifies subtle changes in breathing patterns, oxygen levels, and heart rate trends that could indicate early signs of respiratory decline. For patients with a documented respiratory diagnosis, it's like adding a 24-hour safety net to their care. 

Seeing Ahead: Predictive Accuracy That Saves Lives 

During extensive testing on historical patient data, CareSightAI identified early signs of respiratory decline 3 to 5 days before acute symptoms appeared.

That kind of precision gives our care teams, physicians, and families valuable lead time to act, preventing avoidable hospitalizations and reducing the severity of those that can't be avoided. 

Unlike traditional monitoring, which depends on shift-to-shift notes and observation, CareSightAI continuously learns from weeks and months of patient data. It detects subtle changes that might otherwise go unnoticed, adding another layer of vigilance to the care our nurses already provide.
