The Family LHA Program, nationally referred to as the Family CNA Program, is available for medically fragile families. Through the Family Licensed Health Aide Program, also known as the Family LHA Program, parents and legal guardians of medically fragile children can become Licensed Health Aides to care for their loved ones at no cost to them.

FAQ’s

For those receiving care, they may qualify for LHA care if… 

  • They have chronic diagnosis/medically complex needs. Examples include: 
    • Mechanical Intervention
    • Seizures 
    • BiPap
    • Trach & Vent 
  • They are 21 years of age or younger. 
  • They have qualified nursing hours through DDD – UHC, Mercy Care & TriCare.

Step One: Determine your private duty nursing (PDN) hours. If you do not already have approved PDN hours through DDD, we encourage you to contact your RN District Nurse to schedule a nursing assessment. If you already have approved PDN hours, please contact Team Select Home Care to schedule your Family LHA Program training! 

Step Two: Complete your Family LHA Program training! Contact Team Select Home Care and complete your two-day specialized training based on the unique care needs of your loved one. 

Step Three: Orientation. Regarding your employment with Team Select Home Care, you will need to complete your orientation. This includes the below:

  • CPR certification 
  • Article nine certification 
  • Valid LHA license 
  • Background check and Level I Fingerprint Clearence Card 
  • I-9 documentation 
  • TB testing
  • Team Select Home Care onboarding paperwork 

Step Four: Schedule your Start of Care and begin with Team Select Home Care! 

Private Duty Nursing hours are based on the patient’s medical needs and determined by the DDD RN District Nurse following an evaluation/assessment of the patient. All authorized hours vary from patient to patient based on the medical need for skilled nursing and what care is considered appropriate within the scope of practice for an LHA, as stated by the AZBN.
