At the Luke of Hope Minds Convention in Austin, Texas, our local Austin relationship manager, Jordan, was approached by a smiling family and their nurse… it was little Thomas, one of our Phoenix patients, and his amazing nurse, Kelly! It was a powerful reminder of the importance of pediatric private duty nursing and how we are proud to serve families in 15 states and counting.
Our Model: High-Quality Pediatric Home Care That Scales Locally
Across 15 states, Team Select delivers pediatric private duty nursing (PDN) and family-centered support that meet children where they’re safest and most comfortable, at home. PDN brings licensed RNs/LPNs into the home for skilled, shift-based care that can include respiratory management, tube feedings, seizure precautions, complex medication regimens, and more.
The Team Select Difference
- Consistent quality: Protocol-driven care with nurse mentoring and on-call support.
- Seamless coordination: We align referral providers, hospitals, and schools so care fits your routine.
- Family first approach: Hands on training and routines tailored to mirror real-life situations that help build confidence for the family, not complexity.
Why it matters: Pediatric home health is an effective, family-centered setting for many children with medical complexity that helps reduce hospitalizations and supports a better quality of life for the patient and family members.
Programs and services (what families can expect)
- Private Duty Nursing (PDN): RN/LPN long-term care at home and in the community.
- Family caregiver programs (state-specific): models like Arizona Family Licensed Health Aide (LHA) or state-approved family caregiver options, coordinated through Team Select.
- Skilled Personal Care (where available): allowing patients and their families to select a caregiver who resonates with their preferences.
- Care navigation: Free consultations with our local family care experts to help understanding eligibility, authorizations, and transitions of care.
Many services for children are supported by Medicaid’s state Home- and Community-Based Services (HCBS) waivers that fund care outside institutions. (Eligibility, naming, and coverage vary by state.)
Coverage 101 (plain-English)
- HCBS waivers (1915(c)): States can fund long-term services in home/community settings instead of institutions; pediatric PDN and family caregiver options may be administered under these authorities.
Our dedicated team members help families understand which programs apply in their state and how to get started.
Consistency across states, personalization in every home
Whether your family lives in a large metro or a smaller community, our standards for training, clinical oversight, and communication remain the same… while your plan of care is uniquely yours. That’s how we deliver a consistent experience and a personalized plan across all Team Select locations.
What “high-quality pediatric care” looks like in practice
- Great relationships with hospitals and referral sources
- Structured onboarding and skill matching for PDN nurses
- Clear escalation pathways and after-hours clinical support
- Visit notes that physicians can actually use
