For a client that is entirely new to the waiver program
- Follow the steps listed here.
- Designate someone as your caregiver (please note these individuals cannot be your spouse, legal guardian, or parent of children under 18 - parents of children over 18 are eligible in some cases).
- Have your designated caregiver call Team Select to get started! Their steps will be:
- Apply
- Provide a current TB test from last 12 months. The office will pay for a new TB test!
- Complete orientation and skills competency in our office.
- After your caregiver has been hired, a Team Select representative will schedule a visit at your house to fill out paperwork and get started!
For a client that is already eligible for waiver services and has a caregiver
Contact Team Select to get started! The caregiver will:

- Provide a current TB test from last 12 months. The office will pay for a new TB test!
- Complete orientation and skills competency in our office.
- After your caregiver has been hired, a Team Select representative will schedule a visit at your house to fill out paperwork and get started!
For clients who do not have a family member or loved one to hire as their caregiver
- Team Select will help clients identify someone to care for them, who they may already have a good relationship with.
- If the client does not know anyone, we will work to recruit and hire a caregiver – with the client in mind; however, we want to note that we cannot contractually guarantee staffing.
For caregivers who are looking to be placed with a client
- Please visit our Careers page at www.tshc.com/careers-tshc/ and apply.
- Shortly after, the caregiver will be contacted by one of our recruiters to discuss their experience, availability, and skill set.
- If the caregiver is determined to be a good match for a current client in need or has the flexibility required for PRN employment, an offer will be extended.
For those deemed ineligible either upon application/during annual renewals or clients experiencing a reduction in authorized service hours
Still call Team Select! Individuals who do not qualify for the Aged and Disabled Waiver program in Indiana may still be eligible for services through the Prior Authorization program. Team Select will help clients determine their eligibility for these services or direct to additional resources that could be helpful based on their specific circumstances/approvals.
For a client that is entirely new to the waiver program

For a client that is already eligible for waiver services and has a caregiver

Please note that you must be able to provide your own transportation to and from work and meet a minimum night/weekend availability requirement to be considered for PRN.
