Team Select Home Care, LLC (“Team Select”) collects and uses Personal Information (as defined in the California Consumer Privacy Act of 2018 and the amendments made to the CCPA in the California Privacy Rights Act of 2020) for human resources, employment, benefits administration, health and safety, and business-related purposes and to be in compliance with applicable law and regulation. Below are the categories of Personal Information we collect and the purposes for which we intend to use this information:

Identifiers, such as full name, gender, date of birth, signature, home address, telephone numbers, email addresses, emergency contact information, Social Security number, and dependent and beneficiary information.

  • Preventing unauthorized access to or use of Team Select's property, including Team Select's information systems, electronic devices, network, and data

  • Ensuring employee productivity and adherence to Team Select's policies

  • Investigating complaints, grievances, and suspected violations of Team Select policy

California Customer Records personal information, such as name, signature, Social Security number, physical characteristics or description, address, telephone number, passport number, driver's license or state identification card number, insurance policy number, education, employment, employment history, bank account number, credit card number, debit card number, or any other financial information, medical information, or health insurance information.

Some personal information included in this category may overlap with other categories.

This category of data is used for the same Business Purposes as Identifiers.

Protected classification characteristics under California or federal law, such as Age (40 years or older), race, color, ancestry, national origin, citizenship, religion or creed, marital status, medical condition, physical or mental disability, sex (including gender, gender identity, gender expression, pregnancy or childbirth and related medical conditions), sexual orientation, veteran or military

Biometric information, such as facial recognition, fingerprints, iris or retina scans, keystroke, or other physical patterns.

Limited collection for professional licensure or for systems security access

Internet or other similar network activity, such as your search history, browsing history, login information, and IP addresses on Team Select's information systems and networks.

Sensory data, such as COVID-19 related temperature checks and recording of such temperatures.

Professional or employment-related information, such as your work history, academic and professional qualifications, educational records, references, and interview notes.

  • Investigating complaints, grievances, and suspected violations of Team Select policy

Non-public education information, job title, position, hire dates, compensation, performance and disciplinary records, and vacation and sick leave records.

Evaluating an individual's appropriateness for hire, a participation position at Team Select, or promotion to a new position.

Inferences drawn from other personal information, Profile or summary about an applicant or employee's preferences, characteristics, attitudes, intelligence, abilities, and aptitudes.

Engaging in human capital analytics, including but not limited to, identifying certain correlations about individuals and success on their jobs, analyzing data to improve retention, and analyzing employee preferences to inform HR policies, programs and procedures.

Personal Information collected by Team Select will be retained only as long as necessary to carry out the Business Purposes described in this Notice. Team Select may share Personal Information with third parties, such as background check vendors, third-party human resources and information technology vendors, outside legal counsel, and state or federal governmental agencies in furtherance of these purposes. Team Select does not sell Personal Information.
